The Prologue to Care Coordination
Hello, I’m here to discuss why nurses are so critical in the coordination and continuity of care. While focusing on their patients, nurses should be aware of this crucial competence at all times. According to Capella University (2020), nurses effectively use resources while keeping an eye on factors that impact care coordination. The current objective is for everyone present to be able to contribute meaningfully to care coordination.
Simple Techniques for Collaborating with Patients
According to the Agency for Healthcare Research and Quality [AHRQ], care coordination comprises communicating information throughout the interdisciplinary care group to ensure that patients’ needs are addressed without a hitch (Care coordination 2018). The goal of care coordination is for the patient to obtain optimum health and treatment according to their desires. There are two techniques to attaining integrated care, according to the AHRQ. Broad care, which encompasses cooperation, health information technology, and care management, is the most typical strategy (Care coordination, 2018). An alternative method, referred to as specialized care, focuses on creating a proactive care plan, integrating with community resources, and imparting and sharing information (Care coordination, 2018). Both of these systems take a novel approach to improving patient health and treatment. Johns Hopkins University pioneered a novel method to care coordination known as guided care. This guided care comprises a specially trained enrolled nurse who works in critical care and interacts with essential specialists of care (Boyd, n.d). A well-qualified Registered Nurse makes and directs the patient’s care. Other individuals, such as a physical therapist, a social worker, or a dietician, will be recruited to the group based on the patient’s requirements. According to research, guided care may cut total health costs by 11% while also significantly lowering emergency department readmissions (Catalyst Innovations, 2018). This result is significant since fewer clinic readmissions suggest superior care coordination and patient health.
Change Management and Patient Care
To strengthen patients’ commitment to their care coordination approach, adequate change management is essential. In a medical setting, it is usual for staff to provide patients with a timetable for when they will be discharged from the clinic. This method makes completing all activities very difficult, especially if it entails a change in the patient’s lifestyle. Research shows that patients are more likely to achieve favorable outcomes if they participate in a care plan rather than just being ordered. Furthermore, research reveals that treatment groups may more readily attract patients and help them overcome any hurdles that may arise while changing preferences. Another crucial part of patient care is how healthcare personnel interact with patients. According to research, if communication between the patient and the treatment group is enhanced, the degree of mental flexibility may be raised (Xu et al., 2020). This is crucial because cognitive flexibility may boost a patient’s motivation and ability to overcome obstacles in order to attain the desired outcome. According to the Joint Commission, engaging the patient in the care coordination plan will enhance the patient’s experience and satisfaction. The healthcare business experienced a major upheaval after the passage of the Affordable Care Act. One of the most notable improvements is the Hospital Consumer Evaluation of Healthcare Providers and Systems [HCAHPS], a patient study that integrates consumers’ appraisal of the healthcare organization (Torres, n.d). There are many areas in the HCAHPS summary, including attendant communication, medication communication, discharge information, and overall rating (Torres, 2018). The way patients rate their healthcare providers emphasizes the need to have a sound care coordination system to ensure that the patient receives appropriate communication and discharge planning. The ratings that a healthcare organization obtains as a consequence of these evaluations are closely related to the funds that these organizations would receive if the pay-for-performance principle were implemented (Torres, 2018). If a good care coordination strategy is not in place, the patient experience will suffer, threatening the healthcare organization’s financial survival.
Care Coordination and Ethical Decision-Making
Ethical principles influence nursing and patient care; hence information on these principles is required. Ethical considerations impact how health care professionals (including nurses) set up and operate care coordination systems. The ethical principles of autonomy and beneficence are only two examples of ethical principles that influence a care coordination strategy. Beneficence is when a healthcare organization does something beneficial for others, while autonomy is the patient’s freedom to make their own decisions and be respected. When a patient’s autonomy is respected, this principle allows the patient’s desires to be included in the care coordination plan. This is important since it will enable the patient to make more significant progress after the care plan is finalized. According to Provision 3 of the Code of Ethics for Nursing, “the nurses promote, advocate for, and defend the privileges, health, and welfare of patients” (Code of Ethics for Nurses, n.d). This arrangement highlights nurses’ critical role in ensuring that the patient’s care coordination plan includes the patient and their preferences. Another crucial part of ethical care coordination is cultural practices. This approach ensures that the patient receives the best possible care, regardless of their past experience. According to the ethical principle of fidelity, everyone should be treated fairly and equitably (Davis, 2018). This way of thinking will assist you in avoiding prejudice or humiliation based on the patient’s cultural origin or identity. This is why it is important to foster diversity, which includes a broad spectrum of cultural variations. Another key aspect to remember when dealing with cultural differences is to be aware of how the patient’s culture impacts how they perceive their health and what it means for their future. Nurses should be mindful of how patients engage differently based on their culture, whether it’s due to a lack of visual connection or the making of healthcare decisions. When developing a care coordination plan, these cultural variations should be considered. While the medical attendant is uninformed of these qualifications, it may make a mistake while preparing and implementing the care coordination plan, compromising the patient’s care outcome and overall insight.
In Healthcare Policy Provisions, Outcomes and Patient Experiences
The Affordable Care Act [ACA], as you may have seen, has had an influence on patient care and experience. Parts of the Affordable Care Act (ACA) support increased care coordination. Because of the direct impact on patient experience, the pay-for-performance strategy has pushed clinical education to strongly emphasize patient communication (Torres, 2018). This indicates how crucial communication is for the patient experience and, as a consequence, the overall HCAHPS score of the healthcare organization. When a solid care coordination system is in place, patients will spend less time in the clinic, and many more patients will get follow-up and treatment in the essential care setting rather than the emergency clinic (Andersen et al., 2020). This information is crucial because the patient is more likely to have a positive experience if they get care in the appropriate area, such as an urgent care clinic rather than the crisis division. Remember that the patient experience affects the HCAHPS score, which affects the money received by the healthcare organization.
Nurses play a crucial role in developing and implementing the care coordination approach. When the arrangement interacts with the patient while taking their demands and preferences into preference, there is a rise in significant patient outcomes and general interactions.
